NOTICE: ORDERS MUST BE PLACED BEFORE DECEMBER 1ST TO ENSURE AVAILABILITY
ALL ORDERS PLACED ON OR AFTER DECEMBER 1ST WILL BE SUBJECT TO THE FOLLOWING PENALTIES:

                                         -10% from December 1st - December 31st
                                         -20% from January 1st - January 31st
                                         -30% from February 1st - February 28th
                                         -40% from March 1st - March 31st
                                         -50% on or after April 1st
                         

NOTE:  All orders are pre-paid. Once the below order is received, an invoice will be e-mailed to you. When the invoice is paid, the forms will be shipped out by our distributor. Please note that if payment is not received within 21 calendar days after the invoice is sent, your order will be cancelled.
